An ultrasound guided musculoskeletal injection may be used to target a wide variety of pain conditions.  depending on the condition being treated, the needle will be guided into a joint capsule, within a tendon sheath, or into the tendon insertion or other soft tissue such as a chronic ligament injury. 

Broadly, there are three types of ultrasound guided injection that can be used to help manage musculoskeletal pain conditions:

  • Corticosteroid injections are often delivered with local anaesthetic and provide a rapid anti-inflammatory effect for conditions such as osteoarthritis, inflammatory arthritis, frozen shoulder and other inflammatory tendon sheath disorders.
  • Sodium hyaluronate is a viscous, gel-like substance found naturally within different layers of the body. These injections can be targeted at osteoarthritic joints and tendon sheaths  to reduce friction and increase movement.
  • Chronic tendinopathies are generally not characterized by high levels of inflammation but a failed healing response. Tendon acupuncture/ prolotherapy is a procedure in which our specialist is able to needle the specific site of tendinopathy under ultrasound to kickstart a healing response. This procedure is performed with large amounts of local anaesthetic to achieve the best results.

How is an ultrasound guided injection delivered?

The administration of an ultrasound guided injection is strictly controlled to maximize the effectiveness and minimize risks.

  1. During the diagnostic scan,  your clinician will identify the specific site to be targeted for injection, noting the depth of this region from the skin surface and assessing the most efficient needle path to reach the specific tissue.
  2. The ultrasound probe and the skin around the injection site will be thoroughly cleaned with chlorhexidine wash or alcohol sterets.
  3. Using an aseptic technique to minimize infection risks  your clinician will draw up the doses and quantities of medication required and attach the appropriate needle size and length for the specific injection.
  4. You will be positioned  in a comfortable, well supported posture allowing easy access to the targeted area.
  5. Sterile ultrasound gel will be placed onto the ultrasound probe and the probe will be carefully placed to visualize the structure to be injected.
  6. An injection will be delivered to the targeted area visualizing the needle tip on the ultrasound screen. local anesthetic may be used to numb the area as the needle is passed towards the targeted tissue.

Are there any safety concerns or precautions when having an ultrasound guided injection?

Ultrasound guided injections are an extremely safe procedure by their nature as the procedure will only be undertaken after a careful diagnostic scan of the area. During the ultrasound guided injection the needle tip is visible throughout the procedure minimising any pain and delivering the injectate to the targeted soft tissue or  within the joint capsule. This is in contrast to  unguided injections which have been  demonstrated to be far less accurate with needle placement and medication delivery.

Our in-house specialist will discuss any  potential side effects or specific precautions to be taken on an individual case prior to any appointment or any ultrasound guided injection.  However, the instances of side effects from local anesthetic, corticosteroid or sodium hyaluronate are extremely rare.
